What should I bring for my
appointment?
New patients should
bring their completed Medical and General Information Forms as well as
their current Insurance/HMO Card. Breast patients should also bring
all their mammography and any other imaging studies (Ultrasound, MRI,
etc.) they may have had.
Do I need a referral to see one
of the doctors?
If your Insurance Plan/HMO
requires a referral, you must obtain this from your Primary Care
Physician and bring it with you. The doctor cannot see you without it.

Do you participate in any
insurance/HMO plans?
The doctors at MSA
participate in most of the major plans. See
list. If you have a plan
in which we do not participate, you may choose to use your out of
network benefits to see our doctors.
Is there a co-pay for my visit?
Most HMO plans require a co-pay.
The amount is determined by your plan.
Will I receive any bills in
addition to those from MSA after my surgery?
Patients having surgery in the
hospital will also receive bills from the hospital, the lab, the
anesthesiologist and the pathologist (if tissue is removed and
submitted for analysis). Office surgery patients will receive a bill
from the pathologist if tissue is removed and submitted for analysis.
Coverage of these fees is determined by your plan.
Do you accept any credit cards?
No. Co-pays and any balance due
can be paid by personal check or cash.
How long will it take to
schedule my surgery?
Elective surgery can usually be
scheduled within one to two weeks.

What hospitals are the doctors affiliated with?
Many minor procedures are
done in the office with local anesthesia. More extensive procedures and
those requiring other types of anesthesia are performed at Mountainside
Hospital, Montclair, NJ or Clara Maass Medical Center, Belleville, NJ.
